College admission exam will be waived for First Semester, AY 2021-2022.

However, the grades of applicants will be evaluated according to the admission policy of the University and alignment of SHS strand/track will still be implemented.

ADMISSION PROCEDURES

After downloading the application form, applicants should accomplish the form, affix their signature, and attach 1×1 ID picture.

DOCUMENTS/CREDENTIALS TO BE SUBMITTED

First Year Applicants (SHS graduates)

• • • Accomplished Application for Admission Form
    • Certification of Grades from the latest semester signed by class adviser (downloadable from the online admission system)
    • Grade 12 report card
    • Grade 11 report card (for applicants who will take up Engineering courses)

First Year Applicants (ALS passers)

    • Certificate of rating

Transferees (Those who started college level from other university/school)

• • • Accomplished Application for Admission Form
    • Transcript of records/Certification of grades
    • Other required documents such as Honorable Dismissal, Certificate of Good Moral Character, NBI or Police Clearance shall be submitted AFTER the applicant passed the grade evaluation of the College/Campus concerned

Second Course Applicants (Those who finished a two-year program or four-year degree program)

    • Transcript of records/Certification of grades
    • Other required documents such as Honorable Dismissal, Certificate of Good Moral Character, NBI or Police Clearance shall be submitted AFTER the applicant passed the grade evaluation of the College/Campus concerned

HOW TO SUBMIT THE REQUIRED DOCUMENTS

There are two ways by which applicants may submit all the required documents: hard copy via dropbox/courier/mail OR softcopy via online means.

Hard copy (Submit photocopy of required documents through mail/ parcel/ courier/ drop box addressed to the specific campus of choice)

Online/Soft copy (Submit scanned copy/ screenshot/ clear photo through email/ FB messenger/ google form link)
